Adrienne Rynne, LCSW, C-ASWCM is an Aging Life Care Professional® at Alder, where she has been a valued member of the team since 2022. She holds a Master of Social Work from Adelphi University and is a Certified Advanced Social Work Case Manager (C-ASWCM), bringing extensive experience in aging services to her work with clients and families.

Adrienne has strong care management expertise, including conducting comprehensive assessments; developing and adjusting individualized plans of care; coordinating with physicians, home care agencies, and community resources; advocating within healthcare and insurance systems; monitoring safety, cognition, and functional changes; facilitating communication among family members; and proactively anticipating challenges to ensure seamless, responsive support across the aging journey.

Her professional background spans acute care hospitals, rehabilitation settings, and leadership as a Director of an Assisted Living Residence, giving her a well-rounded perspective across the continuum of care. Adrienne is deeply committed to supporting older adults, their families, and caregivers, helping them navigate complex decisions and achieve their goals with a focus on dignity, independence, and quality of life.
