Amber Koellner has more than 15 years of experience working with older adults and families across healthcare and community settings. She began her career working with children on the autism spectrum before specializing in stroke rehabilitation, geriatrics, and lifestyle redesign across acute care and home health settings. For over 13 years, her work focused on rehabilitation goals and discharge planning in acute hospital care, where she developed deep expertise in home modification and adapting living environments to support safety and independence.

Amber was drawn to care management because it gave her the freedom to practice the kind of comprehensive, unhurried care that traditional clinical settings rarely allow. She brings a strong clinical foundation alongside a natural affinity for deep human connection. She is genuinely curious about the people she works with, and committed to partnering with them on their own terms.
