Chloe brings two decades of healthcare experience across assisted living, skilled nursing rehabilitation, adult day care, and art and recreation therapy, which gives her a perspective on aging care that most clinicians simply don't have.

After years in healthcare leadership, she felt a pull toward something more direct: working alongside the families and individuals actually navigating chronic illness, care transitions, and long-term planning. Care management is where that instinct found its home. Her approach centers on meeting people where they are: listening actively, adapting to what each client needs, and building the kind of trust that makes difficult conversations feel manageable. Outside of work, Chloe spent twelve years as a competitive ice skater.
