Danielle Westerman brings nearly two decades of experience in geriatric care, including 12 years as a Certified Occupational Therapy Assistant in skilled nursing facilities — with a focus on home safety evaluations and aging in place — and seven years as a Rehab Director overseeing family care plan meetings, discharge planning, and care level transitions.

She came to care management drawn by the directness of the impact: assessing a client's circumstances, and then building a plan to help them genuinely thrive. For some clients, that means showing up consistently as a steady, knowledgeable presence and becoming a trusted advocate who ensures nothing goes unnoticed and nothing falls through the cracks. That kind of hands-on, ongoing involvement is what she finds most meaningful about the work. When she's not working, Danielle enjoys weekly games of golf and Mah Jong.
