Elizabeth Pond is a clinical social worker with 15 years of experience across skilled nursing facilities, short-term rehabilitation, and hospice and palliative care. That range of settings has given her a deep understanding of what families encounter at each stage of aging and serious illness, and the weight of the decisions that come with them.

She came to care management because it allows her to combine clinical expertise with something she is genuinely passionate about: helping people move through uncertainty with confidence and dignity. She specializes in dementia care, advance care planning, adjusting to new diagnoses, and supporting families through complex medical decisions. What she brings to every client relationship is a commitment to truly listening first — understanding each person's circumstances, values, and concerns before offering guidance.
