Emma Fortin brings over 15 years of experience working with older adults and individuals with disabilities across community and healthcare settings. She previously served as Community Discharge Planning Specialist Supervisor at Springwell, Inc., where she managed complex psychosocial and medical discharges and oversaw implementation of MassHealth programs including the Frail Elder Waiver, Personal Care Attendant Program, and Money Follows the Person Program. Her background includes roles as Clinical Consultant for Social Work in Progress providing clinical supervision in skilled nursing facilities, and Director of Social Services at Wingate at Andover. She holds a Master's degree in Social Work from Boston College with a clinical concentration in Older Adult and Families. Emma is an Advanced Professional Level member of the Aging Life Care Association and serves on the Board of its New England Chapter.
