Grace Ambrose
Grace Ambrose is an occupational therapist whose clinical background spans acute rehabilitation, assisted living, home health, and outpatient settings, with a focus on supporting older adults through complex medical, orthopedic, and life transitions. Throughout her career she has worked closely with clients, families, and interdisciplinary teams to promote independence and help people navigate the healthcare challenges that often accompany aging.
She came to care management because it brings together what she finds most meaningful: clinical problem-solving, advocacy, and genuine relationship-building. She is skilled at seeing the whole picture — identifying what a client needs, coordinating the right resources, and staying connected to the person at the center of it all.
