Jillian Gibbons is a Licensed Physical Therapist with eleven years of experience in long-term care, skilled nursing, and dementia care. Her background in interdisciplinary collaboration, care coordination, discharge planning, and home safety gives her a practical, whole-picture view of what it takes to help older adults live safely and well at every stage.
Jillian came to Aging Life Care Management because she wanted to go beyond treating a condition — she wanted to support the full person, and the family around them, through the complexity that comes with aging and change. A natural planner with a sharp eye for the details that others might overlook, she has a particular gift for anticipating what's coming before it becomes a problem and responding with both clarity and compassion. Outside of work, Jillian is a loyal Patriots and Celtics fan!
