Nicole Mott is an occupational therapist who has built her career across both the clinical and leadership sides of healthcare. She spent years as a Lead OT in nursing homes, acute care, and acute rehabilitation before moving into Director of Rehabilitation roles across nursing home and home care settings.

Nicole came to care management because she wanted to make an impact that extended beyond a single patient encounter or facility. Having worked as both a frontline therapist and a rehabilitation director, she brings a perspective on the healthcare system from multiple vantage points — which means she can anticipate what a client is likely to face, not just respond to what's already happening. She advocates fiercely for the families she works with, with the confidence that comes from having navigated that system herself, from the inside. Outside of work, Nicole channels her energy into home organization, staying active with sports, and hosting pool days for family and friends.
