Patty brings experience across some of the most demanding settings in healthcare, including home health, hospice, palliative care, and hospital care management, giving her a clinical foundation that is both broad and deeply practical. She understands what complexity actually looks like in aging and serious illness, and she knows how to navigate it.

What draws Patty to care management is the relationship at the center of the work: the opportunity to know a person, understand what matters to them, and help them live as fully as possible within their circumstances. She is a skilled communicator, a clear-headed advocate when a crisis requires one, and someone who brings genuine knowledge of community resources to every family she works with. Thirteen years ago, Patty donated a kidney to the son of a colleague. He was 26 at the time. He is doing well.
